Dementia/Alzheimer’s Care
We do not boast a secured Alzheimer’s unit but do provide trained staff to care for our Alzheimer’s and Dementia patients. Our secure facility boasts SecureCare on the 2 main floors.
Restorative Nursing Program
At Chandler Creek Post Acute we seek to maximize the quality of life for our long-term care population who require a specialized restorative program. The therapy team and nursing staff develop individualized treatment plans for resident care in the areas of:
- Ambulation
- Range of motion
- Activities of daily living
- Positioning and alignment
- Skin integrity and wound management
- Contracture management
- Bowel and bladder training
- Rehabilitative and social dining
- Strengthening and restraint reduction
- Weekly and monthly height and weight monitoring
Pain Management
We understand that pain can delay and interrupt the progress of rehabilitation. We strive to make our patients as comfortable as possible, through medication and other pain management techniques, to help patients participate in and benefit from therapy. We also provide our patients with educational information and resources they will need to successfully manage temporary, long-term, or chronic pain once they return home.
Dietary Program
- Provides meals to meet specific dietary and therapeutic needs, according to physician orders
- Oral supplemental diets are available
- Regular nutritional assessments from our registered dietitian
