I prayed for a miracle and God gave me Chandler Creek. I cannot say enough good things about the care team there. Gwen was an absolute angel on our arrival. Thanks so much Chandler Creek for all you did for my family!

The meals have not improved and residents are always hungry, the dining room has swarms of flies, the residents don’t have access to their monthly cash allowance-they need $1 to use for drink and snack machines. These are the most pressing issues, there are too many to mention.

I could write a book on the issues endured at Chandler Creek Post Acute facility – from lack of proper nutrition to neglect of basic needs during night shifts. Please do not allow anyone you know be sent to Chandler Creek Post Acute. My Daddy started getting nauseous, weak and altered at times on Wednesday. By Friday he was VERY sick after a weak performance in therapy. Previously, the Nurse Practioner was sunshine personified. Always chipper and singing. However, when she heard that I needed My Daddy sent to the hospital she became aggressive and combative. She would not listen to my concerns. She continually spoke over me stating again and again “There is no medical reason for discharge”. All of the staff knew that I had power of attorney due to Daddy’s dementia. The NP charged in with a second NP to accost my Daddy asking him “do you know what your daughter is doing?” and stating “if you leave here, you cannot come back”. Is this the way to treat an 87 year old patient with dementia and is on his third sick bag? The Chandler Creek NP refused discharge him to the hospital. I had to call EMS myself and I had to sign an Against Medical Advice document. The hospital conferred with me about current medications stating that Chandler Creek did not send over the proper documents. After tests were performed at Pelham Medical Center, My Daddy was immediately admitted with pneumonia. He is receiving breathing treatments, IV antibiotics and fluids. It is expected that he will remain in the hospital for at least several days receiving treatment for the pneumonia. Additionally, blood and urine samples are being monitored for possible “growth” of infection. The hospital’s main concern is UTI because of his cloudy output as well as worsening confusion and weakness. Update on Monday My Daddy has a drug resistant viral infection. He has the dreaded door hanger full of protective gear that the staff will have to wear going forward. Perhaps the NPs at Chandler Creek will add pneumonia and drug resistant viral infections to their list of “medical reasons” for discharge to the hospital. The first items on their “to do” list should be listening to the family member’s concerns and NOT threatening 87 year old sick patients who have served their country and deserve respect.

Nice clean facility. The food is delicious. It is the best nursing home that i have been in south Carolina. The staff is on point and we’ll organized. The only bad thing about the facility is that it shorted out two of my phone chargers. The staff did keep men out of my room that i didn’t want in my room. The best facility I have been in in south Carolina. Very clean and organized. This is all of the time. Unlike many other nursing homes. I also did not have to watch my things from getting stolen. I would definitely recommend this facility to anyone.

If there was a. Zero I would give it to them them !!!!! I do not know where to start. From medical negligence to breaking the law with own home meds that are control substances. I left there in an ambulance to the ER and told them I was never come there again and they did not send my meds so they broke the law. They were even suppose to count my meds in front of me and I was suppose to witness when they received them and when I left so I am reporting this and the negligence to the everyone. I got no dinner when I got there. Next day sold them what I couldn’t eat and got the wrong meal. So I didn’t eat then again. Was given the trash can to vomit in. And there is more. Just do not go there!!!!

If I could give this place NEGATIVE stars, I would. Most of the nurses, with a few exceptions, have terrible attitudes and neglect the patients. Especially patients on hospice! They double diaper my Grandma! Her room is always dirty, her bed is dirty, her bathroom, etc. Her clothes/stuff go missing all the time. THEY HARDLY DO THE BARE MINIMUM. This place should be meticulously inspected and certain people should be fired.

Staff is very pleasant, kind, and caring. I knew from the 1st time I called that my uncle was taken care of. The staff nicknamed him Sarge and they share life experiences with my uncle to make his stay nice and they appreciate his stories as well. My only concern is that I’ve been trying to reach my uncle since Thanksgiving and I cannot seem to be connected to him; either no one answers the nurse station he’s recently been moved to or I’m on hold over 30min+ to no avail. Everything else has been great. Administrative staff and front desk staff have beautiful hearts.

I can’t say enough positive things about Chandler Creek. My mother had a long battle with three hospitalizations and two rehabs. Her first experience was with NHC and they did not listen to my concerns or keep me informed. The day she was released from there home health had her readmitted to the hospital. I chose Chandler Creek because it was close to my home what I really didn’t expect was the amazing care she received. The Nurse Practitioner listened to my concerns and found the UTI that was causing confusion in my mother. She also had a heart surgery and knee replacement all in three months so the therapist worked with her to make sure she could walk again. The staff listened and really cared about the patients. I watched my 80 year old mother hug those nurses and CNAs so I know she was treated kindly. The admittance was easy and the entire staff was always willing to help. I honestly can’t thank those people enough for what they did.

While the staff have been very cordial, I am upset that since entering this facility on 9/9/2025 my husband has not received a shower as per THEIR. Schedule. I’m very concerned about this lack of attention to his personal hygiene. I guess I’ll have to do it myself! Better get those towels ready folks.

I volunteer for one of the hospices and I am a retired Activity Director. Walking into a nursing home can be dicey experience. You don’t know where you are headed. I got zero attitudes from the staff. Everyone I saw, although swamped busy, was friendly and took a moment to give me directions on the room locations I needed to visit. This was directly after lunch. I saw smiles on the staff, pleasant attitudes, and smiles on the residents. Signs that show a happy work environment. Thank you for doing such a wonderful job and Happy Nurses Week!

As a nurse that visits many facilities, this one is beyond great!! Matt and his clinical staff strive hard to provide care to each patient as they deserve. It’s a really clean place and filled with a lot of smiling faces!

I have reached out on multiple occasions and multiple different ways to get the contact to their human resources and the administrator of the building due to the falsifying of information from the individual over their nursing staff without any response from anyone I have attempted to speak to.

Our family has had an exceptional experience with Chandler Creek Post Acute. The team here has gone above and beyond in assisting us, particularly with navigating the complexities of the Medicaid process. Their expertise and support have been invaluable during this time. The facility itself is outstanding—clean, secure, and very well-organized. It is clear that the staff takes great pride in maintaining a high standard of care and professionalism. Every person we have encountered has been incredibly friendly, knowledgeable, and compassionate, creating an environment that feels welcoming and supportive. Because of the excellent care my father has received, we are confident in the decision for him to transition to long-term care here. Chandler Creek Post Acute has truly exceeded our expectations, and we are deeply grateful for the peace of mind they have provided our family. I highly recommend this facility to anyone seeking skilled rehabilitation or long-term care for their loved ones.
